The PPO dentists agree to accept reduced fees for covered procedures when treating PPO patients After the annual deductible of $35 (the deductible is reduced to $25 if you use Delta PPO dentists), the plan pays 80% of the cost of covered preventive and basic services and 50% for major services, up to a calendar year maximum of $3,000. Major services include crowns, cast restorations, bridges and dentures. For orthodontia services (braces) the plan pays a lifetime maximum of $4,000 per dependent child.
